Changing Futures Stoke-on-Trent: The Final Year

As we enter the final year of the programme, we continue our commitment to embarking on System Change to ensure a more adaptive approach to individuals facing multiple disadvantage.

Our recruitment is currently underway to compliment our existing team within Adult Social Care, which will see an addition of a Senior Social Worker and 2 Social Workers, enabling swift Care Act Assessments for the cohort and providing holistic, flexible solutions to prevent crises via the utilisation of personal budgets. We also have additional recruitment to bring a Case Manager and Case Co-Ordinator to the team.

This will enable additional support to the MaRG (Multi-Agency Resolution Group) and increase capacity to support more individuals. The conclusion of the MaRG evaluation will enable us to integrate recommendations to enhance our existing model and further improve our partnerships with senior stakeholders.

The role of the MaRG Independent Chair has been viewed very positively, and will continue. We are keen to embed the principles of lived experience throughout the programme and the system overall, from both a workforce development and frontline support perspective, which will see the continuation of the Peer Support Recovery Co-Ordinators and Insight Academy. Our Case Co-Ordination Team and Support Worker role will continue, based within Adult Social Care alongside the Project Management Team, Case Managers and Strategic Manager.

We are keen to retain the knowledge and impact of the Citizens Advice Bureau (CAB) and will be continuing to work on the programme to provide support to our staff and customers to enable the maximisation of their income.  An independent evaluation currently being conducted will enable more evidence to demonstrate the effectiveness of the programme and engage with our partners to explore potential funding opportunities to enable the sustainability of Changing Futures in Stoke.
