Changing Futures Sustainability Plan

The Changing futures Stoke-on-Trent headline goal remains fundamental to the work the team will undertake during the Project lifecycle:
To focus on customers who are entrenched in multiple disadvantage proving resistant to interventions, intervening to break cycles of chronic disadvantage and prevent recurrence. For those who have fallen into multiple disadvantage recently, we will work to upstream intervene and prevent people being entrenched.
The Changing Futures Programme is a three year programme funded by the Department for Levelling Up Housing and Communities and The National Lottery Community Fund.
The Stoke-on-Trent Project commenced slowly and as a result, DLUHC and The National Lottery Community Fund have required a Sustainability Plan to enable the funding, and ultimately the Project, to be extended by one year.
The Sustainability Plan has been developed in partnership and collaboration with all key stakeholders including Board members and was submitted on 27th January 2023 – we are currently awaiting the outcome from this.
The revised Delivery Plan will be based on key learnings from the Project to date and the core team will be meeting in May 2023 to review the service, outcomes, feedback from customers and lived experience to ensure the remaining project focuses on what matters to our customers and will give the best outcomes for them.
The Plan will also seek to secure additional funding and commitment from the sector and partners to ensure the key principles and learning that impact on service provision and outcomes remain a fundamental element of supporting and enabling people experiencing multiple disadvantage to receive coordinated care that meets their needs in a timely manner, giving respect and dignity and wraparound care to enable people to live fulfilling lives – The aim is to identify and remove gaps in service provision, maximise expertise and coordination of care and resources.
